چکیده

SIR, A double-contrast arthrographic technique was used in the first six frozen shoulder patients studied, but no advantage was found over the single-contrast technique, and the latter technique was used in the remaining patients. These details were unfortunately omitted from the final draft of the paper. However, the procedures were performed under x-ray control, and adequate joint outline was obtained with both methods. There was no difference in the rate or extent of recovery of pain or range of passive movement and no difference in the frequency or severity of side effects with either technique. We therefore stand by our observation which is, as Wright and Haq' suggested, that arthrographic features are not consistent in patients with painful stiff shoulders.

Still's disease associated with Coxsackie infection and haemophagocytic syndrome.

At the onset of Still's disease (systemic-onset juvenile arthritis) in a 12-year-old girl, serological evidence of Coxsackie B virus infection was found. Two weeks later she developed a haemophagocytic syndrome which was then treated with cytotoxic therapy. Her arthritis is still active six years later.

Still's disease and the virus-associated haemophagocytic syndrome.

A 15-year-old male developed features of Still's disease. He was treated, with incomplete response, with aspirin and corticosteroids. Some 10 months after the onset he contracted chickenpox and became gravely ill. A diagnosis of histiocytic medullary reticulosis was made because erythrophagocytic histiocytes were detected in marrow aspirate films. Necropsy studies showed systemic infiltration b...

Patients with an extraordinarily elevated serum ferritin: think of haemophagocytic lymphohistiocytosis.

BACKGROUND We retrospectively analysed charts of patients with blood ferritin level >5000 µg/l. The aim of the study was to look for the likelihood of haemophagocytic lymphohistiocytosis (HLH) in these patients. METHODS Forty-two patients demonstrated hyperferritinaemia and could be evaluated. The diagnosis of HLH was based on a recently published HScore and an earlier diagnostic algorithm. ...

Haemophagocytic syndrome in rheumatic patients. A systematic review.

BACKGROUND Hemophagocytic lymphohistiocytosis (HLH), is a potentially fatal hyperinflammatory syndrome characterized fever, hepatosplenomegaly, and cytopenias. HLH can be either primary, with a genetic aetiology, or secondary, associated with malignancies, autoimmune diseases, or infections. Among rheumatic disorders, HLH occurs most frequently in systemic juvenile idiopathic arthritis. AIM T...

Refractory Adult Onset Still’s Disease

It is often a challenge and a dilemma for clinicians encountering patients with pyrexia of unknown origin. Numerous tests performed to determine the underlying cause often give inconclusive results. We present a 52-year-old man with undulating fever for more than 10 months with persistent hyperferritinaemia, and negative immunological and serological markers. Despite corticosteroids, disease mo...
